One of the simplest and most important things you should do is to create an I.C.E. (In Case of Emergency) list for all family members. It should include the following information:

  1. Doctors’ names and phone numbers
  2. Parents’ birthdates
  3. Insurance information
  4. List of allergies
  5. Blood type
  6. Major medical problems
  7. Medications
  8. Prior surgery
  9. Advance directives/health care proxies
  10. Emergency contacts and their relationship to the patient

A copy should be printed and displayed somewhere prominent (such as on the refrigerator or by a phone). You and any close relatives, friends and neighbors should also have copies. Another good idea is for each family member to keep a small, laminated version of their I.C.E. list in their wallet next to their driver’s license.

If your family members have cell phones, their I.C.E. numbers should also be programmed there. If there is more than one contact, label them either ICE 1, ICE 2 etc, or ICE Sally, ICE Joe.

Your family members may have more than one residence, and may have doctors in each area. Prepare I.C.E. information for all residences.
